PATIENT FORMS

If you are new to our office, please visit our patient forms page to fill out our new patient forms that will need to be filled out prior to your arrival at our office. If you're unable to fill them out electronically you can print them and bring the forms to the appointment filled out. Thank you and please contact our office if you have any questions.

DENTAL INSURANCE

Dental insurance is a great benefit to have to help with the costs of dental treatment. If you do have dental insurance, we would be more than happy to file it in our office. For your particular insurance, call our office for a complimentary benefits check. Please do not hesitate to ask any necessary questions so that you can have a full and complete understanding of your dental benefits. A financial/insurance agreement will be presented to sign before any initial visit with our office. We also offer a VIP Office Membership which can be explained thoroughly by a team member for those who do not have dental insurance. 

PAYMENT OPTIONS

We can accept all major credit cards, as well as cash or check. Our office also accepts CareCredit, and if you’re unfamiliar with this option we would be more than happy to give any information on it. We also offer a VIP Office Membership available for purchase – please call our office for details and pricing.
